Portability Form

Are you planning to request number portability? Fill out this form and bring your numbers to UZO.

To complete the request, you'll need:

  1. Enter the UZO account holder's tax ID number;
  2. List up to 4 cell phone numbers for number portability;
  3. Attach the required documents (up to 8 files):
  • For mobile numbers:
    Mobile Number Portability Form (Instructions for Completing the Form)
  • Identification document for the holder of each number

All form fields marked with an * are required.

You can attach up to 8 files in JPG or PDF format, with a maximum total size of 10 MB. To select multiple files from the same folder, use Ctrl + click (Windows) or Command + click (Mac). You can also combine them all into a single ZIP file.
